New Patients


When you visit us for the first time, we want to make sure that your experience with us will create a long and lasting relationship filled with healthy smiles.

Medical History
You will be asked to fill out a patient questionnaire providing us with a brief medical history and dental background in order to make sure that we are providing you with the best care possible. We want you to remain healthy and safe during your procedures. That is why it is important for you to provide us with any medical information that may affect the outcome of your procedures.

We will need to know about medications you are taking, allergies that you may have, and any serious illness you have experienced in the past. Please bring your health insurance information and a list of the prescriptions and supplements you are taking.

Your Initial Exam
Plan to arrive 15 minutes before your scheduled appointment to fill out the new patient forms. When that is complete, the dentist will perform an initial examination to determine your dental health. We will take x-rays so that we can properly examine the areas of your teeth and gums that are not visible.
Once your assessment is complete, we will work together with you to determine your care requirements and the best way to bring you the Big Happy Smiles you deserve.
